Albany Associates in Cardiology is seeking an Advanced Practitioner to join our team specializing in Heart Failure care. Position Overview:
As an Advanced Practitioner in Heart Failure, you will play a key role in the management and care of patients with complex heart failure. You will work closely with the cardiology team to assess, diagnose, and develop comprehensive treatment plans for patients, ensuring optimal outcomes. This role requires a compassionate and knowledgeable individual who is passionate about heart failure management and improving patient quality of life.
Responsibilities:
- Perform comprehensive patient assessments and diagnostic evaluations related to heart failure.
- Develop and implement individualized care plans, including pharmacologic and non-pharmacologic interventions.
- Monitor patient progress, adjust treatment plans as needed, and provide ongoing education to patients and families.
- Collaborate with cardiologists, nurses, and other healthcare professionals to provide coordinated care.
- Order and interpret diagnostic tests, including echocardiograms, EKGs, and lab work.
- Participate in patient rounds, case discussions, and follow-up care.
- Provide patient education regarding heart failure management, lifestyle modifications, and medication adherence.
- Stay current with the latest research and advancements in heart failure care.
- Document all patient interactions and maintain accurate and up-to-date medical records.
Qualifications:
- Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Previous experience in cardiology or heart failure management preferred.
- Strong clinical knowledge and the ability to manage complex cardiac conditions.
- Excellent communication skills and the ability to work well within a multidisciplinary team.

Recruitment Package:
